Pay attention to what your knee does each day. Stiffness, swelling, and lost motion can guide your next choice. The answers below cover common concerns.

What usually helps knee pain first?

Easy motion and steady strength work help many knees. Take short walks. Try chair rises if they don't cause sharp pain. You can do less during a flare, but don't stop all movement unless a clinician tells you. Topical medicine means a cream or gel rubbed on your knee. Ask your doctor whether it fits your other medicines.

Is walking good for a sore knee?

Usually, yes. Keep the walk within what your knee handles. Mild soreness can happen, but it shouldn't keep rising. Shorten your next walk if swelling or limping follows. Add time slowly once the next morning feels steady. Walking can't solve every cause. A knee that locks, buckles, or stays swollen needs an exam.

What can help a knee called bone on bone?

The X-ray is only part of the choice. Your sleep, walking, balance, and pain at rest matter each day. Exercise, weight loss, a brace, or short-term care may help you move. They won't rebuild worn cartilage. Replacement may be worth discussing when severe limits continue. Ask what each treatment can truly improve.

What does common knee arthritis feel like?

It often causes stiffness after sitting or sleeping. Your knee may loosen after a few steps. Longer walks, stairs, or standing from a chair can bring aching. Swelling may come and go. You might notice less motion or a grinding feel. The soreness won't always match the X-ray. A clinician can check whether arthritis is the cause.

Do I need an MRI for knee soreness?

Not every sore knee needs one. Your health history, an exam, and an X-ray may give enough information. An MRI can help if the result would change your care. Locking, repeated buckling, or a major injury may make it useful. Ask what the scan would find. If care won't change, the scan may add little.

High-quality evidence from 44 trials (3,537 participants) shows land-based exercise reduced knee OA pain by an equivalent of 12 points on a 0-100 scale (SMD -0.49) immediately after treatment, with moderate-quality evidence for a ~10-point function gain; the effect attenuated but persisted at 2-6 months (SMD -0.24). No serious adverse events were reported in any included trial.

In the 68-week STEP 9 trial, 407 participants with obesity (mean BMI 40.3) and moderate knee OA with at least moderate pain were randomized 2:1 to once-weekly semaglutide 2.4 mg or placebo alongside diet and activity counselling. Weight change was -13.7% vs -3.2%, and WOMAC pain improved -41.7 vs -27.5 points (both P<0.001). Gastrointestinal adverse events drove discontinuation in 6.7% vs 3.0%.

A 2026 systematic review and meta-analysis of eight sham-controlled RCTs (n=627) found genicular nerve ablation (radiofrequency or cryoneurolysis) reduced knee OA pain versus sham at 12 weeks (MD -1.65; 95% CI -2.57 to -0.74) and improved WOMAC function (MD -11.37), with high heterogeneity (I2 83-91%) and no serious adverse events reported. The population studied was patients INELIGIBLE for arthroplasty.

  6. The RESTORE trial randomized 288 adults aged 50+ with symptomatic mild-to-moderate medial knee OA to three weekly leukocyte-poor PRP injections or saline placebo. At 12 months, pain change was -2.1 vs -1.8 points (difference -0.4; 95% CI -0.9 to 0.2; P=.17) and medial tibial cartilage volume change was -1.4% vs -1.2% (P=.81). 29 of 31 prespecified secondary outcomes showed no significant between-group difference. The authors concluded the findings 'do not support use of PRP for the management of knee OA.'

A 2026 network meta-analysis of 21 RCTs (2,254 patients) found both leukocyte-poor and leukocyte-rich PRP significantly improved 6-12 month WOMAC function versus placebo (MD -10.54 and -13.20 respectively) and both were superior to hyaluronic acid, with leukocyte-poor PRP ranked first (P-score 0.96) — a materially more favourable read of PRP than the RESTORE trial, which is why this corpus presents both.

In a randomized trial of 100 patients with moderate-to-severe knee OA eligible for total knee replacement, TKR followed by 12 weeks of non-surgical care improved the KOOS4 score more than the same non-surgical programme alone (32.5 vs 16.0 points; adjusted mean difference 15.8, 95% CI 10.0 to 21.5) — but produced four times as many serious adverse events (24 vs 6, P=0.005), and 26% of the non-surgical group had chosen surgery within 12 months, meaning roughly three-quarters had not.

FDA states verbatim: 'None of these products have been approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' The only FDA-approved stem cell products in the United States are blood-forming (hematopoietic progenitor) cells derived from umbilical cord blood, approved only for disorders of blood production. There are currently NO FDA-approved exosome products.

  10. A 2026 Cochrane review searching from 2010 to January 2025 found only ONE randomised trial (100 participants, Denmark) comparing knee arthroplasty with non-surgical treatment. Low-certainty evidence indicated TKA may reduce pain at one year by a clinically important margin (MD 17.60, 95% CI 8.25 to 26.95) and may improve function by an amount that might not be clinically important (MD 12.40), with probably no clinically important difference in health-related quality of life.
